What Causes Hand, Wrist, Foot & Ankle Pain?

Plantar Fasciitis
Plantar fasciitis is one of the most common causes of heel pain, caused by inflammation and degeneration of the plantar fascia — the thick band of tissue connecting the heel bone to the toes.
Patients typically experience sharp heel pain with their first steps in the morning that usually improves with movement but returns after prolonged standing. It most commonly affects one foot and is prevalent among runners, people who are overweight, and those who wear shoes with inadequate support. A 2024 systematic review and meta-analysis published in PMC found that targeted injection therapy is more effective than corticosteroid injections, extracorporeal shockwave therapy, and placebo in reducing pain and improving function for plantar fasciitis. A separate 2022 study comparing targeted injection therapy to partial plantar fasciotomy surgery found the injection therapy group recovery averaged 10.2 months compared to 37.2 months for the surgical group, with lower complication rates in the injection therapy cohort.

Chronic plantar fasciitis can lead to altered gait patterns, which in turn may cause secondary knee, hip, and back problems.
Trigger Finger & Hand Conditions
Trigger finger (stenosing tenosynovitis) occurs when the tendon sheath in a finger becomes inflamed, causing the finger to catch or lock when bent.
Patients experience a painful clicking or snapping sensation, stiffness especially in the morning, and sometimes a palpable bump at the base of the affected finger. In severe cases, the finger may become permanently locked. Initial treatment with steroid injection has approximately 90% success rate, with 66.3% of patients needing no further treatment. However, 30.5% require re-injection within 6 months and 3.2% proceed to surgery within 1 year. Hand and wrist osteoarthritis, particularly at the base of the thumb, causes pain, stiffness, reduced grip strength, and difficulty with fine motor tasks. Bony enlargements known as Heberden's and Bouchard's nodes may develop. Evidence-based injection treatments and regenerative approaches are being studied for their potential to support tendon healing and reduce inflammation in these conditions.

Trigger finger may progress from occasional catching to permanent locking. Hand OA can lead to joint deformity and severe functional limitation.
Ankle Tendon & Ligament Injuries
Ankle tendon injuries, particularly Achilles tendinopathy, involve pain and stiffness along the Achilles tendon that is typically worst in the morning and worsens with activity.
The tendon may become swollen and thickened. Ankle ligament tears from sprains can lead to chronic instability if not properly rehabilitated. Published research has explored targeted injection therapy for Achilles tendon conditions, with studies showing improvements in pain intensity and functional ability. One study found that implanting advanced injection approaches into Achilles tears in athletes improved range of motion and function more quickly than open suture repair. Level 4 evidence also showed no reruptures at 2.5 years after open Achilles repair augmented with non-surgical injection therapies. Ankle osteoarthritis, while less common than knee or hip OA, causes pain, stiffness, and reduced range of motion that can significantly affect walking and daily activities. Targeted injection therapy may support healing in soft-tissue ankle injuries.
